    • FD 200mm F2.8: 7.5meters
    • FD 200mm F1.8: unknown (infinity seems possible with lens calibration, not tested yet)
    • FD 300mm F2.8L: 29m (infinity with lens calibration)
    • FD 300mm F4.0L: unknown (infinity with lens calibration but there is an extra pretty hard step to do it)
    • FD 400mm F2.8L: 50m (infinity with lens calibration)
    • FD 400mm F4.5: 85m (near infinity possible with lens calibration)
    • FD 500mm F4.5L: unknown (infinity with lens calibration)
    • FD 500mm F8.0 Mirror: infinity (by easy removal of a stop screw under the rubber focus grip)
    • FD 600mm F4.5: infinity (removal of a stop screw in the nFD version's gear housing helps maintain infinity in low temp shooting - reportedly not needed in SSC versions)
    • FD 800mm F5.6L: infinity (removal of a stop screw in the nFD version's gear housing helps maintain infinity in low temp shooting - reportedly not needed in SSC versions)
  • The conversions easily done with the kits above are fully reversible.

General Notes:
1.) Do by no means take this list as complete. It's not possible to collect such a list and it wouldn't make any sense to do so. This list is meant to show some examples of what kind of adapters are possible and widely available. Also I might say, that all high quality adapters are indeed included (that's Cameraquest, Kindai, Novoflex, SRB-Griturn and Zörk). An additional collection of worldwide suppliers can be found here.
2.) Adapting lenses to the Canon EOS 5D and 7D models is reportedly more risky than with other EOS models due to the bigger mirror of these bodies. Lists of incompatible lenses can be found at CameraQuest, Kindai (see EOS adapter pages) and Panoramaplanet.
As also described for Ed Mika adapters, at least the 5D II and III can be operated in Live View Mode (mirror is locked), which should be the easiest and most economic way to deal with this problem. I think the same applies to the 7D (FD lenses mounted via Ed Mika adapters do not collide with the mirror, that's why he doesn't state it there). For the 5D some camera repair shops might offer professional modifications. Ask your local dealer.
In Germany for example contact M. Richard Wiese Fototechnik (Hamburg, 9hrs, ~650€) or Hartblei (Lindenberg, 600€).

Footnotes:
1 simulates a lens in manual focus mode, so the camera can find a lens and will show an AF mark within view finder
2 the aperture pin of auto lenses is pressed into the lens by the adapter ring, so working aperture does not work
